Mi Ayala Circuit Makati Grand Opening e-Raffle Promo

  1. Customer must purchase a minimum single-receipt of P5,000 at Mi Ayala Circuit Makati on July 29, 2022.
  2. Raffle draw is on July 30, 2022, Saturday.
  3. To qualify for the raffle, customers must register at https://forms.gle/83fXQhk1k4z3eMz68 on or before July 29, 2022, 11:59pm.
  4. Participants must fill out the necessary details. Only valid and successful registrations will be qualified for the raffle draw:
    • Customer’s Full Name
    • E-mail Address
    • Mobile Number
    • Receipt Number
  5. All qualified entries during the promo duration will be drawn on July 30, 2022.
  6. A total of 3 winners will win from the raffle promo:

  7. All winners can only win once.
  8. The winning customer will be notified by SMS and e-mail. Digitalks Technology Corp. reserves the right to disclose and publish names, photos and other information of the winners in any mode or manner as they deem appropriate.
  9. To claim the prize, the winner must present the winning receipt and at least 2 government issued IDs (i.e., Driver’s License, Passport, SSS, TIN, Voter’s ID etc.) to the designated store representative of Mi Ayala Circuit Makati branch during claiming of prize.
    • Should the prize be claimed thru an authorized representative, the representative must bring the following when claiming the prize:
      • Winning Receipt
      • Authorization Letter with the winner’s signature
      • Photocopy of at least 2 valid government-issued IDs of the representative and
      • Photocopy of at least 2 valid government-issued IDs of the winner
      • Prizes cannot be exchanged for cash, credit, or products other than the prizes indicated above.
      • All winners will only have seven (7) calendar days to redeem their prize upon receipt of the SMS and e-mail. All unclaimed prizes will be forfeited in favor of Mi Ayala Circuit Makati. Mi Ayala Circuit Makati shall not be liable if the winners did not receive the SMS and e-mail due to outdated personal information.
  1. In case of loss of official receipt, it will not be replaced or reissued. Mi Ayala Circuit Makati cannot be held liable for official receipts that were lost, mutilated or defaced and such damage that prevents the representative from identifying the security and authenticity features thereof.
